Stem cells, conditioned medium, fibroblasts: similar names, three different things
These three terms take turns appearing on price quotes, and the prices can differ by a factor of dozens. The difference is not how "premium" they are. It is whether you are buying the cells themselves, the fluid the cells leave behind, or an entire culturing process.

You have three quotes in hand. One says "stem cell IV drip," one says "conditioned medium skin-booster injections," and one says "autologous fibroblast transplantation." Everyone who described them called it "regenerative medicine," in much the same tone, so the only thing left to compare is the price. The most expensive one is probably the best, right?
That reasoning doesn't hold here. These three names are not three tiers of the same thing. They are three different things. Telling them apart takes about five minutes, and it may save you from buying the wrong one.
1. First, separate the three terms
Stem cells. In aesthetic and regenerative medicine, the most common type is adipose-derived mesenchymal stem cells: a small amount of your own fat is taken, the cells are expanded at a cell-processing facility, and then they are returned to your body. The core variables you are paying for are the number of living cells and the number of infusions. That is why price lists for these treatments are tiered by "how many hundred million cells per session."
Conditioned medium. This is the one most easily confused. While stem cells are cultured, they release various substances into the culture fluid. The fluid left after the cells are separated out is the conditioned medium (sometimes called "stem cell supernatant"). The key point: it contains no cells. It is not the same thing as stem cells, and it is not a "budget version of stem cells." It is a different category altogether.
Fibroblasts. These are not stem cells. They are cells that naturally live in the dermis of your skin. A small piece of your own skin is taken, the fibroblasts in it are cultured, and they are then transplanted back into the area of skin you want to improve. By nature, their effect is local.
2. The price list alone shows they are not the same thing
You don't need to hear the sales pitch. Line up how the three types of treatment are priced, and the difference jumps out. The following comes from one facility's July 2026 price list that we have, with all amounts as listed in yen, tax included:
Stem cells (IV infusion): priced by cell count and number of sessions.
The 100 million cells per session tier is JPY 3,355,000 for one session; the 150 million cells per session tier is JPY 3,465,000 for one session; and the 200 million cells per session tier is JPY 4,400,000 for one session. More sessions bring tiered discounts, and packages are valid for 2 years from the date of purchase.
Conditioned medium: priced by the vial (A) and by treatment area.
Conditioned medium IV drip is JPY 55,000 for 1A and JPY 110,000 for 2A; a 4x concentrated conditioned medium injection is JPY 165,000 for 1A. For local injections, a joint (one knee) is JPY 66,000 for 1A per area, or JPY 198,000 at 4x concentration; targeted injections to the shoulder, lower back, neck and so on are the same price (both shoulders count as two areas). For skin-booster injections, stem cell conditioned medium 1A plus hyaluronic acid for the full face is JPY 99,000 for one session and JPY 267,300 for three; switching to 1A plus Pluryal Densify is JPY 165,000 for one session and JPY 445,500 for three. Scalp injections are JPY 88,000 for 1A, or JPY 237,600 for a 3-session package.
Fibroblasts: charged in three parts, and none of them works without the others.
First you pay JPY 594,000 for "skin harvesting plus initial culture" (enough culture for 10 sessions). After that you are charged by the volume transplanted: JPY 330,000 for 1cc, JPY 759,000 for 4cc, JPY 1,518,000 for 8cc and JPY 1,897,500 for 10cc. If the cells are to be stored long term, there is an annual storage fee of JPY 198,000 per year. In addition, anyone who tests positive on the infectious disease screening pays a separate JPY 33,000.
Do you see it? The pricing units for the three types are "cell count × sessions," "vials × areas," and "culture + cc volume + annual storage." Different pricing units mean they are not selling the same thing at all. So using price to judge which one suits you asks the wrong question from the start.
3. Treating the whole body and treating one area are two different decisions
Set the names aside, and the three types separate clearly by where they are delivered:
- IV infusion is a whole-body (systemic) route of administration.
- Local injections (skin boosters, joints, targeted points, scalp) act on the area that is injected.
- Fibroblast transplantation puts cells back into the patch of skin you want to improve. It is local by nature, and there is a culture period in between.
So the question to answer first is not "which one is stronger," but this: do you want to change one specific area, or your overall condition? The first points to a local approach, the second to a systemic one. You can answer this yourself, and once you do, half the lines on your quotes drop away.
While we're at it, here is a trap people fall into every year: the three types need completely different amounts of time. Local injections are usually done in a single visit. Treatments that involve harvesting and culturing require a wait between sampling and transplantation. If your trip is only three to five days, this kind of treatment simply cannot be done on this trip.
4. A few things we have to say
First, the "list price" on a price list is the price filed with the regulatory authorities. Filing is a compliance procedure, not certification of effectiveness. When you see that line, please don't read it as "officially proven to work."
Second, the strength of the published evidence differs greatly across the three types, and it cannot be transferred from one to another. Claims about one type cannot be used as support for another. Even within the same type, different uses (for example, IV infusion versus joint injection) cannot be carried over directly.
Third, we will not tell you what results a certain number of sessions will bring. Individual differences, your baseline condition and your lifestyle all affect the outcome. No facility can guarantee results for an individual, and any claim that does should itself be treated with caution.
5. Next step: re-sort your quotes by these three distinctions
You can finish this today without contacting anyone:
Take out each quote you have and ask four questions about every line. Put a question mark next to anything you can't answer. Those question marks are what to ask at your next consultation.
- Is this item giving you cells, the fluid the cells leave behind, or something else?
- What is the pricing unit? (Hundreds of millions of cells / sessions, vials / areas, or culture fee + cc volume + storage fee)
- Is it systemic, or does it act on one area?
- What needs to happen before treatment? This step is often skipped: an initial consultation fee of JPY 5,500, a regenerative medicine consultation fee of JPY 11,000 and a regenerative medicine blood test of JPY 33,000 (if ovary-related treatments are assessed at the same time, the combined blood test is JPY 44,000, which is JPY 22,000 less than doing them separately). All are July 2026 prices, tax included. Together they come to a tiny fraction of a single treatment, yet they are the necessary step that gives the big number afterward a basis.
